Some house rules we use (3.5):

1. If you have at least a +1 BAB, you have the quickdraw feat.
2. All players get 1 reroll a session (their own rolls only). Additional rerolls can be earned by good role-playing.
3. No evil characters. We're here to play heroes, dammit!

I'm sure there's more - just can't think of them right now...

Noteworthy acts (eg a paladin holding off the creatures so his party can get to safety, a rogue taking a risky path to an alternate exit, creative/out of the box use of spells, a druid using his skills to preserve nature rather than to combat evil, etc)...pretty subjective...

I have used something similar to this for years. We called them Hero Points and you got 1 at 1st level and any others had to be earned.I also allow them to be used to force a bad guy to reroll on you as well.
